• Measuring instruments – patients use these to regularly check their health data.
• Base station – this independently transmits the vital parameters to the Telehealth platform and enables communication with the patient.
• Telehealth platform – the data service, which centrally records and processes the patient data, whilst maintaining data security.

The telemedical caregiver can easily access the processed data on screen. Using a partially automated process to support the medical diagnosis and identify the risk potential, the caregiver or can analyse the data and, at the next step, control the therapy management. With regular control, the caregiver is able to reduce acute situations.
